"The application of ossoeintegrated dental implants has became one of viable treatment options for rehabilitation of partially edentulous patients. However, the poor bone quality and Insufficiency of bone volume limited implant placement in posterior maxilla area. To overcomethese problems, maxillary sinus augmentation procedure has been used to provide morepredictable success of implants in the severely atrophic maxilla.
The purpose of this study was to evaluate the long-term clinical outcomes of various treatmentoptions which may affect the predictability of the maxillary sinus augmentation.
Two hundred sixty one implants (of 77 patients) placed in the posterior maxilla were investigated up to 76 months after implant installation and restored with implant supported fixed
prostheses. The data were collected and analysed by implant type, diameter, gender, age andsinus lift procedures. Kaplan-Meier survival curve and log rank test were used for life time table and statistical analysis respectively.
Within the limits of this study, following results were drawn.
1. The cumulative survival rate (CSR) of sinus lift procedure with bone graft in the delayed fixture installation group was 92.3%. The CSR of sinus lift procedure with bone graft in the immediate fixture installation group was 64.0%(p<0.05).
2. The CSR for conventional implant installation procedure without sinus lift for machined surface implant was 92.2%, which the bone grafted group resulted in 82.3% of (p<0.05). There was no significant differences in the survival rates between the surface treated groups.
3. There was a significant difference in CSR between the conventional procedure and sinus lift with bone graft procedure for 4/4.5mm diameter implant fixtures.
4. The CSR of autogenous bone graft group resulted in 88.9%, which the CSR of mixed graft group with autogenous bone and allogenic bone was 70.3% (p<0.05).
5. The survival rates by the gender and age were not significant in the cumulative survival raters(p>0.05)"
