The maxillary segmental osteotomy is for reconstruction of skeletal deformities. The posterior maxillary segmental osteotomy has several other indications. It may be used to close gaps in the alveolar process when teeth are missing, or to correct asymmetries in the arch form in any direction. Miniplates, but especially microplates are almost indispensable for stabilizing the fragment. The clinical practice of oral and maxillofacial surgery has always involved management of a wide variety of facial bone deformities. The two basic indications for surgical correction of an isolated unilateral transverse discrepancy by widening the maxilla are when the teeth within the affected maxillary segment are well aligned, no crowding exists, and the shape of the segment is such that the indicated surgery can be performed shortly after placing the orthodontic appliances as determined by feasibility model surgery.
Patient has multiple teeth fractures and missing, mandible communicated complex fractures. He has severe right posterior vertical dimension loss. Traditional posterior maxillary segmental osteotomy improved the patient status, resulting in undercorrection and compromised aesthetic results with many implants. And we are now treated segmental osteotomy and implants and improved predictable results. In this report we review our indications for maxillary segmental osteotomy, implant restoration and describe our technique using after mandible open reduction.
