The aim of this case report was to evaluate the result of anterior implant restoration under complicated situation and to consider the advantage of guided bone regeneration with soft tissue graft. There is much alveolar ridge resorption with soft tissue atrophy in all cases due to periodontitis, peri-implantitis and external root resorption. When the bone morphology allows anchorage of the implant with primary stability in correct position, the one-step approach was preferred. In situations where the defect morphology precludes primary implant stability, the two-step procedure was performed.
