The authors observed the bone responses to the osseointegrated implants with different surface characteristics such as, Bonfit®(titanium plasma sprayed(TPS) hollow-screw type), IntegralR(hydroxylapatite(HA)-coated cylinder type), Steri-Oss®(HA-coated screw type), and Steri-Oss®(uncoated smooth surfaced screw type) in the mandible of dogs.In 3 adult dogs(20-25 kg B.W.), The mandibular 2nd, 3rd, and 4th premolar teeth were removed bilaterally. Four mon ths later, 4 kinds of implants were placed in the edentulous sites. The dogs were sacrificed on 4, 8, and 12 weeks and radiographs were taken, followed by tissue block containing. The block sections were fixed in 70% alcohol for 3 days, and suspended in Villaneuva staining solution for 3 days and embedded with polymethylmethacrylate. We evaluated the bone reactions on undecalcified ground specimens under light microscope.
The obtained results were as follows:
1. After 4 and 8 weeks, TPS or HA-coated implants had less fibrous tissue contact and more bone formation than un coated implants. HA-coated implants were most favorable in the degree of osseointegration.
2. Bone tissue were substituted for osseous fibrous connective tissue adjacent to the surface of the implants and the implants were surrounded with compact lamellar bone after 12 weeks.
3. Bone contact with the surface of the implants was more intimate and bone remodeling was faster in the cortical bone than in the medullary bone.
4. Bone ingrowth into the porous surfaced implants was faster than the smooth surfaced implants.
5. Radiographically, there were no significant differences between implants after 8 weeks. However, bone growth over
the implants was observed in HA-coated implants after 12 weeks.
The results form this study demonstrate that the hydroxylapatite coating provides an osteophilic surface for bone deposition and the porous surface allows more rapid bone ingrowth, but there are no significant differences depending on surface characteristics with time over a 12 week period.
