Although surgeons try to install fixtures with surgical stent, it is not always the case to place fixtures at the planned sites. In such situation taking impression in a conventional procedure sometimes becomes a burden. In this case report resin index was utilized for the three mal-aligned fixtures. This method is often called “Index impression technique.” 3 fixtures of 3.75 mm in diameter, 13, 10, 7 mm in length were placed on the right mandibular molar region. The fixtures were shown to be convergent occlusally on standard X-ray. Contact among impression copings prevents taking impression of three fixtures together, so impression was made in two stage. Impression of the first two fixtures was made with square impression copings. And the position of the 3rd fixture was registered using resin index. Because of the angulations among fixtures fabrication of one-piece metal framework was not possible. UCLA abutments were used to splint the 2nd and 3rd fixtures through custom-made copings. Coping for the 1st fixture was also custom-made independently. Outer crown was fabricated to splint the 1st and 2nd copings. Through the 4 years follow-up this prosthesis was well used and there was almost no alveolar bone resorption. Well-planned resin index will be a good help for solving mal-positioned fixtures and other use.
