One of the biggest clinical problems associated with osseointegrated implant prosthesis is the excessive stress caused by lateral bite force. The study was aimed at analyzing the stress distribution of implants and supporting tissues relative to the placement of implants. A 3-dimensional finite element analysis model was made at the maxillary 1 st premolar, the 2nd premolar and the 1 st molar region. Type Ⅲ gold alloy was used for the crown materials. The three implants were placed as follows.
Model Ⅰ: straight line
Model Ⅱ: 1st & 3rd implants - 1mm palatal offset (from straight line), 2nd implant - 1mm buccal offset
Model Ⅲ: 1st & 3rd implants -1mm buccal offset, 2nd implant - 1mm palatal offset
Tests have been performed at 300N on the lingual incline of the buccal cusp at the 2nd premolar.
The stresses of implants and supporting tissues were so also analyzed.
1. Major stress was concentrated in the cortical bone around the neck of loaded implants.
2. Buccal and palatal offset placement resulted in stress reduction at the cortical bone and 1st thread of the implant.
3. Buccal offset was more effective than palatal offset at the maxillary partial edentulous region.
4. The stress difference is minimal at the cancellous bone.
